After reading her book, Augustus is disappointed that it ends abruptly with no resolution as if Anna had passed away unexpectedly. 

Hazel explains that after the book’s release, Peter van Houten withdrew to Amsterdam and hasn’t been seen or heard from since.

She claims that steroidal treatment has given her “chipmunk cheeks.” She has metastases that have spread to her lungs and stage IV cancer.
